Mask and Puzzle Locations with Pictures

So right of the bat I can say I got most of this information from the "Hotline Miami Wiki"[hotline-miami.wikia.com] I took the screenshots and provide simple instructions on somethings, but most credit goes to the wiki.

If you're complaining that this game is too hard and hard to control PLAY IT WITH AND XBOX CONTROLLER. In the recent patch they added controller support so take advantage of that because I can ensure you that it makes the game so much easier. Comment if you need help. There's lots of ways to get A+. To get them really quick look for an enemy that only has a melee weapon and just keep knocking him down it gives you points and another obvious way is to get longer combo chains with Brandon (Panther) or Zack (Frog).

The X marks on chapters stand for the mask you will get at the end of the chapter if you hit a certain score. They all so stand for the puzzle pieces in chapters. So if you play a chapter and you see 2 X marks it means you haven't found the the puzzle piece and that you haven't earned enough points for chapter mask. In other situations you only have earn the mask and there's a X mark next to the mask it means you haven't found the puzzle piece in the chapter.

In Full House (Chapter 5) Requires multiple steps. After your done with the chapter pick up a crowbar (it's red) and don't go in your car instead walk around the house in till you see a sewer or manhole and it will take you down to this guy and next to him is the Jones mask.

On the body in the disco room in Push It (Chapter 8), right as you enter the level

Jake (Snake): Killing Throws
On a body in Hot&Heavy (Chapter 10)

Found in the Assault (Chapter 13), upper right corner of the level with the police chief and the prisoner, on the table.

Charlie (Octopus): More Melee Weapons
Top left corner of the level, on the counter in the kitchen area of Safehouse (Chapter 16)

Phil (Fish): French Translation
When you do a chapter that has dialog while your wearing the mask the mask translate the english dialog in to french.

No Talk (Chapter 1) "T": Top left corner of the second floor, in the kitchen

Overdose (Chapter 2) "A": Bottom right corner of the second floor, in the bathroom next to toilet

Decadence (Chapter 3) "O": Bottom left corner of the second floor, in the dining room.

Tension (Chapter 4) "A": Left side of the second floor, in the bomb room close to the window.

Full House (Chapter 5) "E": Bottom right of the first floor, in the kitchen area, right of the chairs.

Clean Hit (Chapter 6) "I": Bottom right of the second floor, on the balcony, close to the second bush from top.

Neighbors (Chapter 7) "N": Top left corner of the first floor, in the bed room, next to the table.
Push It (Chapter 8) "R": Top right corner of the rightmost second floor, in the disco room.

Crackdown (Chapter 9) "S": Top middle of the second floor, right next to the elevator from which the Swat enters from. I messed up and forgot to take the image but it's next to that elevator

Hot&Heavy (Chapter 10) "I": The floor which entrance is the top right door from the base map, bottom left room with deck chairs and pool in it, next to a chair.

Deadline (Chapter 11) "N": Bottom left of the second floor, in the dark room, below the desk next to wall.

Trauma (Chapter 12) "W": Bottom right of the floor you wake on, in the bathroom next to the toilet.

Assault (Chapter 13) "S": Bottom left corner of the first floor, in the bathroom, next to toilet.

Vengeance (Chapter 14) "B": In the bathroom of third floor that you see right as you come up the stairs, top left next urinals.

Showdown (Final Chapter) "H": Left side of the first floor, between the sofa and the chair next to wall.

To get the secret ending and unlock the Nigel (Bat) mask you need all the puzzle pieces once you get all the puzzle pieces hit [esc] on your keyboard or [back] button on your Xbox 360 controller and go to the puzzle board and put [IWASBORNINTHEUSA] after that get out of the menu and that lets you use the computer in chapter 19 "Resolution"
